Needed Forms Before You Come to Camp

First and foremost, overnight campers, read our overnight acceptance package for more information. After you have registered for Camp, we need the successful completion of these forms:

CAMPER AND CIT MEDICAL FORM

This form provides the camp the pertinent medical history of all campers and CITs. Importantly, it requires an attached record of a physical exam within the last 2 years from your licensed medical provider. Please plan accordingly to prevent any issues during registration.

CAMP CODE OF CONDUCT

The implementation of rules at the Middlesex County Camp are created to foster a safe and wholesome environment, with an underlying goal to provide a rewarding and enriching summer camp experience for campers. Parents, guardians, campers, and CITs need to review and agree to abide by the code of conduct to attend the Middlesex County Camp.

An important note from the Code of Conduct: campers who bring cell phones to Camp will be sent home.

DAY CAMPERS ONLY: CORE CHOICE FORM

As overnight campers select their core program areas on Sunday night, we request that day campers to submit the Day Camper Core Choice Form prior to their arrival Monday morning. This form will be used to assign day campers into their preferred cores program areas.
